Completing the Pot hole claim form – San Diego, CA (English)

At American Factory Wheel, we know that filing the paperwork for pothole damage to your car or truck can be just as annoying as hitting the pothole itself! No worries, we’re going to walk you through it so you can get it done easily and get on with your life. Here we go.

1. Click on the link to open the document
2. Click on the gray boxes under the heading “Claimant Information” and fill in your Last Name, First Name, Middle Name, Street Address, City, Zip, etc.
3. Do the same under the heading “Claim Information” and fill in the gray boxes with the information. (This section continues to the second page.)
4. On the second page, fill out the last three headings, “Damages Claimed”, “Damaged Vehicle (if applicable)”, and “Additional Information”.
5. Read the promissory note at the bottom of the page and double check that the information in your claim is accurate and true.
6. Fill in the gray boxes in the last line of text on the page with the day, month, year, and time that you filled out this document.
7. Write your signature on the line above the words “Claimant’s Signature”.
